The Record Review Process

Whether you are interested in taking the first step toward having surgery with Dr. Redwine, or you simply want one of the world’s leading endometriosis experts to review your personal case and evaluate your treatment options, the process is simple. It starts with a request for Dr. Redwine to review your medical records. Please follow the guidelines below.

Please send your records (see Guidelines for Record Evaluation below) directly to Dr. Redwine's office. Please DO NOT send this information to St. Charles MedicalCenter – Bend. Once Dr. Redwine has reviewed your records, he will send you a letter detailing his observations and recommendations. His office will also provide general cost estimates for your case, should you be considering surgery here.

If you were only seeking an expert opinion, that ends the process. You are free to share Dr. Redwine’s letter with your regular physician to decide on the best course of treatment for your case.

If, after reviewing Dr. Redwine’s recommendations you have further questions, or you would like to schedule surgery, please call Dr. Redwine's office directly at 541-382-8622. His staff can provide further cost and insurance information as appropriate and explain travel and lodging options. Dr. Redwine is unable to accept patients with Medicare or Medicaid.

Guidelines for Record Evaluation
NOTE: ALL the following must be received by Dr. Redwine's office before he can start a record evaluation. 

  • Documents and Materials Essential for a Record Evaluation:
  • Operative and pathology reports if you have had pelvic surgery; ( Authorization to Release Information Document )
  • A personally written history (this should be in your own words, and should include a brief description of your pain: when it occurs, duration, location and a basic overview of physical symptoms you experience and treatment history).  You might want to use this pain diary to help you create your history. http://www.reliefinsite.com/
  • A copy of current office dictations from your gynecologist, if available;
  • Please do not send x-rays;
  • Appropriate fee as noted below.

Fees:
New Patients/Inquiries: There is a fee of $125 (U.S) for a record evaluation. Please send payment with your request and materials. The $125 fee is non-reimbursable by insurance companies as a second opinion. If you choose to have surgery with Dr. Redwine, the $125 will be applied toward your surgical bill with Dr. Redwine.

For Prior Patients:
There is a fee of $50 (U.S.) to review the record of a patient that has had surgery with Dr. Redwine in the past. This fee can be applied to your out-of-pocket should you return for another surgery here. If you are a former patient and you would like Dr. Redwine to review your current medical record, please write him and enclose any appropriate medical reports (operative/pathology/ ultrasound reports and current gynecological dictations).
