The Record Review Process: Whether you are interested in taking the first step toward having surgery with Dr. Redwine, or you simply want one of the world’s leading endometriosis experts to review your personal case and evaluate your treatment options, the process is simple. It starts with a request for Dr. Redwine to review your medical records. Please follow the guidelines below or click here to begin your record review now with our online form.

Record Review Guidelines: Please Note : ALL of the following must be received by Dr. Redwine's office before he can start a record evaluation.
- Operative and pathology reports if you have had pelvic surgery: Authorization to Release Information Document
- A personally written history (this should be in your own words, and should include a brief description of your pain: when it occurs, duration, location and a basic overview of physical symptoms you experience and treatment history). You might want to use this pain diary to help you create your history. http://www.reliefinsite.com/
- Office dictations from your most recent gynecological exam.
- Current Pelvic Radiology Reports (Ultrasound, MRI or CT exams). Do not send X-Rays, CDs or DVD's.
- $125 fee: this fee will be applied to your surgery deposit should you decide to have surgery with Dr. Redwine.
Once we have all of your record review information, Dr. Redwine will review your case. Generally you will receive your Record Review Packet within 2 weeks. The packet will include a detailed letter from Dr. Redwine with his recommendations. Included with Dr. Redwine's letter will be information regarding costs and insurance.

Due to their fee setting policies, Dr. Redwine is unable to accept patients with Medicare, Medicaid or Oregon Health Plan. If you have one of these insurances you may still wish to obtain a record review to share with your doctors at home.
